Please note:

  1. Only REQUIRED MATERIAL should be placed on reserve.
  2. Reserve loan periods for print materials may be 2 HOURS, 1 DAY, 2 DAYS, or 3 DAYS.
  3. If an item is a PERSONAL COPY, please check the appropriate box. Remember that faculty members are responsible for bringing personal copies to Green Library.
  4. This form will allow eight reserve items to be submitted at a time. If you have more to submit, you will be given the option to fill out another form.
  5. PLEASE NOTE: DO NOT MIX PRINT RESERVE MATERIALS FOR DIFFERENT CLASSES ON THE SAME FORM. You can submit an unlimited number of reserve materials for each class, but always go to a new, blank page when starting the reserve list for a different course.
